In exchange for the consideration of my job application by Taylor Smith Consulting, LLC (hereinafter called "the Company"), I agree that:
Neither the acceptance of this application nor the subsequent entry into any type of employment relationship, either in the position applied for or any other position, and regardless of the contents of employee handbooks, personnel manuals, benefit plans, policy statements, and the like as they may exist from time to time, or other Company practices, shall serve to create an actual or implied contract of employment, or to confer any right to remain an employee of Taylor Smith Consulting, LLC or otherwise to change in any respect the employment-at-will relationship between it and the undersigned, and that relationship cannot be altered except by a written instrument signed by the President /General Manager of the Company. Both the undersigned and Taylor Smith Consulting, LLC may end the employment relationship at any time, without specified notice or reason. If employed, I understand that the Company may unilaterally change or revise their benefits, policies and procedures and such changes may include reduction in benefits.
I authorize investigation of all statements contained in this application. I understand that the misrepresentation or omission of facts called for is cause for dismissal at any time without any previous notice. I hereby give the Company permission to contact schools, previous employers (unless otherwise indicated), references, and others, and hereby release the Company from any liability as a result of such contract.
I also understand that (1) the Company has a drug and alcohol policy that provides for pre-employment testing as well as testing after employment; (2) consent to and compliance with such policy is a condition of my employment; and (3) continued employment is based on the successful passing of testing under such policy. I further understand that continued employment may be based on the successful passing of job-related physical examinations.
I understand that, in connection with the routine processing of your employment application, the Company may request from a consumer reporting agency an investigative consumer report including information as to my credit records, character, general reputation, personal characteristics, and mode of living. Upon written request from me, the Company, will provide me with additional information concerning the nature and scope of any such report requested by it, as required by the Fair Credit Reporting Act.
I further understand that my employment with the Company shall be probationary for a period of ninety (90) days, and further that at any time during the probationary period or thereafter, my employment relation with the Company is terminable at will for any reason by either party.
Although, it is our goal to help you find full-time, permanent, employment; Taylor Smith Consulting does not guarantee a position of permanent employment with any of our clients. The clients of Taylor Smith Consulting may offer permanent employment opportunities within their company at their own discretion. Taylor Smith Consulting does not have any authority over the decision making and hiring process in place with its clients.
Upon completion of your temporary employment assignment, it is your responsibility to contact Taylor Smith Consulting's Administrative office within 3 business days to notify the office of your availability to work. Upon ending a temporary employment assignment with Taylor Smith Consulting, there is no promise or guarantee of unemployment compensation benefits. If you fail to contact us, it could jeopardize your eligibility of unemployment compensation benefits.

I have received a copy of Taylor Smith Consulting, LLC’s Employee Safety program policy. I agree to follow the rules it contains. I also understand that failure to comply with this program could result in disciplinary action, up to and including termination. I understand that it is my responsibility to consult with the Human Resource Department should I have any questions regarding the safety program policy.
I consent to a drug or alcohol test as a condition of my initial employment or continued employment. I authorize any laboratory or medical provider to release such test results to the company.
In the course of performing your duties, you may have access to or gain knowledge of confidential information concerning the Company, its customers/clients, and other employees. "Confidential information" is defined as information to which the public and other employees within the company does not have general access. This policy governs the use or disclosure of such information.
With respect to confidential information concerning the Company, other employees, the Company's vendors and contractual partners, and/or its customers/clients, such information should be safeguarded, not shared, and not discussed with anyone. An appropriate manager will grant the necessary access if an employee needs such information to perform his or her duties. No other access is permitted. Any release, duplication, distribution, transmittal, disclosure, or discussion ("release") of such information that is not required by law or by the duties of your employment is strictly prohibited.
Unauthorized access to, discussion of, and unauthorized release of, confidential information will violate this policy and will result in disciplinary actions, up to and including termination of employment, depending upon the severity and/or nature of the offense.
Signing below acknowledges that you, herein referred to as EMPLOYEE, agrees to the terms and conditions of the agreement stated above.
